鉤翼夫人者，
齊人也，
姓趙。
少時好清淨，
病臥六年，
右手拳屈，
飲食少。
望氣者云：
「東北有貴人氣。 」
推而得之。
召到，
姿色甚偉。
武帝披其手，
得一玉鉤，
而手尋展，
遂幸而生昭帝。
後武帝害之，
殯屍不冷，
而香一月間。
後昭帝即位，
更葬之，
棺內但有絲履。
故名其宮曰鉤翼。
後避諱，
改為弋廟。
闈有神祠、閣在焉。
婉婉弱媛，
廟符授鉤。
誕育嘉嗣，
